ICBC pays the clinic a set rate per visit; the clinic’s own listed price is often higher, and the gap is the user fee. It is legal, it varies clinic by clinic, and almost nobody publishes it — which is why the only fees on this page are the ones clinics wrote down themselves. The other constraint is the clock.
