When you arrive for your appointment, please check in using our online system at summitrmt.janeapp.com/checkin and wait in your car or outside until you are greeted.

All patients are required to wear a mask . Please bring your own if you have one, otherwise one will be provided for you.

Due to the risk of transmission of COVID-19, we ask that all patients who are sick or experiencing symptoms such as fever, cough, sore throat, runny nose, sneezing, shortness of breath or difficulty breathing, unexplained fatigue, nausea, vomiting, diarrhea or abdominal pain ,
