Intake Form & Card on File:

All new patients are required to complete an intake form and have a valid credit card on file prior to their first appointment. This allows us to reserve your time and streamline your visit.

We require at least 24 hours’ notice for any cancellations or appointment changes. This gives us enough time to offer your spot to another patient.

Late Cancellations & No-Shows:

If less than 24 hours’ notice is provided, or if you do not show up for your appointment, your card on file will be charged an appointment fee .

“I came in months ago dealing with concussion symptoms and Chantal Simak was able to help through vestibular physio sessions. She's incredibly knowledgeable but also one of the warmest, most understanding people you'll meet, which makes a huge difference when you're dealing with something as frustrating as concussion recovery. She’s genuinely one of a kind. She does a really thoughtful mix of vestibular/concussion rehab, dry needling/IMS, and neck work, and she takes the time to actually explain what's going on and why she's doing what she's doing. Highly recommend Chantal for physio! Also, been seeing Dr Dan for chiro the last few months, highly recommend as well! He’s very knowledgeable and helped a lot with my recovery.”

“Nicole one of the Administrative assistance always greets me with a friendly smile. Troy the practionner that i see on a regular basis , is not your regular massage therapist. After a treatment from Troy, my arthritic pain has significantly decreased and i have mobility back. “I first visited Dr. Daniel Zybutz due to acute back pain. After a few adjustments, I fully recovered! Later, I experienced bicep tendinitis and was in pain for four months. In January, I returned to see Dr. Zybutz, who performed shock wave therapy and adjusted my neck, shoulder, and back several times. I regained 90% of my shoulder movement and can now play pickleball again. Thank you very much, Dr. Zybutz! I highly recommend him!”

“I brought my daughter here for chronic neck pain and headaches after two sports related concussions. After seeing her improvement I booked my own sessions with Dr Dan. I was amazed at how thorough the initial session was, charting my posture, taking neck and spinal X-rays and going over them with me immediately. After two sessions the hip pain that has plagued me for two years is virtually gone. My vertigo is gone and my neck is more relaxed than it has been in years.
